Responsibilities
- Perform comprehensive nursing assessments in patients’ homes and document findings
- Develop, implement, and adjust patient-centered care plans in collaboration with the interdisciplinary team
- Administer medications and treatments per physician orders; monitor patient responses
- Provide skilled nursing interventions such as wound care, IV therapy, and routine monitoring
- Educate patients and families on disease management, medications, safety, and discharge planning
- Communicate with supervisors and care teams, ensuring accurate documentation and timely updates in the patient record
Requirements
- California RN license (active) required
- CPR or Basic Life Support (BLS, HS-BLS, or RQI BLS) required
- Valid driver’s license; at least 21 years of age
- Ability and willingness to travel to patient homes within the service area
- Mileage is reimbursed in addition to the bill rate
