Responsibilities
- Administer medications and treatments per physician orders; document accurately in the resident charting system.
- Monitor residents’ conditions, perform focused assessments, record vital signs, and escalate changes to the supervising RN or physician.
- Provide direct care for activities of daily living, including bathing, dressing, feeding, and mobility assistance.
- Support wound care, ostomy care, and other routine nursing tasks per plan of care under supervision.
- Maintain safety, infection control, and device/therapy maintenance; respond promptly to call lights.
- Collaborate with the interdisciplinary team to update care plans while preserving resident privacy and dignity.
Requirements
- Active Colorado LPN license
- Graduation from an approved practical nursing program (LPN diploma or equivalent)
- 1+ year of recent experience in long-term care or skilled nursing preferred
- Ability to work night shifts and maintain high-quality patient care across 12-hour blocks
- Proficiency with electronic health records and nursing documentation
- Strong communication skills and ability to work within a multidisciplinary team
