Responsibilities
- Provide skilled nursing care to Home Health patients per physician orders and the plan of care
- Perform comprehensive RN assessments, observe patient status, and document findings
- Administer medications, manage wound care, and perform therapies within scope; support IV therapy where permitted
- Complete OASIS assessments and ensure accurate data collection and documentation
- Develop and update plans of care in collaboration with the interdisciplinary team; coordinate with case managers and physicians
- Educate patients and family members on disease management, safety, medications, and self-care techniques
- Maintain patient safety, infection control, and compliance with agency policies; ensure timely documentation of visits
Requirements
- Active New Jersey RN license
- 2+ years home health experience
- OASIS experience
- Availability to work the listed home health setting with one weekend out of every four
- Local candidate or true traveler if commuting over 40 minutes to the territory
