Responsibilities
- Perform comprehensive speech-language pathology assessments for residents in an acute care context within a skilled nursing facility
- Develop and implement individualized treatment plans addressing communication, cognitive-communication, and swallowing impairments
- Provide direct therapy, monitor progress, and adjust plans to meet established goals
- Document evaluations, treatment sessions, and progress notes in the facility’s electronic medical record
- Collaborate with an interdisciplinary team (nursing, medical staff, and therapy colleagues) to optimize patient outcomes
- Participate in care planning meetings and educate residents and families on strategies for communication, swallowing safety, and daily functional communication
Requirements
- Active license to practice Speech-Language Pathology as required by the assignment
