Travel Licensed Practical Nurse Long-Term Care Job
Licensed Practical Nurse – Long-Term Care in Claremont, New Hampshire. This travel assignment starts on 04/28/2026 for a 13-week contract in a long-term care setting. Responsibilities
- Monitor residents’ mental and physical status continually; report changes to the Supervisor and discuss physician notifications; provide a detailed hand-off on shift change.
- Accurately receive, transcribe, and implement physician orders; administer medications and treatments per orders; document via the electronic MAR/TAR system.
- Order and maintain unit pharmaceuticals; supervise LNAs per NH Board of Nursing scope; assign and evaluate care, assisting direct-care staff as time allows.
- Accompany physicians on rounds on assigned units to address medical needs and facilitate communication for 60-day reviews.
- Administer oxygen and procedures such as gastrostomy/jejunostomy feedings per policy; perform wound care and manage wound vacs when applicable.
- Conduct routine unit duties (therapy appointments, activity boards, ambulation lists, nutrition documentation); monitor LNA documentation and infection control practices; maintain a hazard-free unit.
- Admit, discharge, and transfer residents; complete required documentation and notifications; prepare 24-hour unit reports and coordinate with the Nurse Manager/Nursing Supervisor.
- Manage controlled substances per regulations; participate in staff orientation, continuing education, and performance management as needed.
Requirements
- Active NH Licensed Practical Nurse license
- BLS (AHA) certification
- IV certification (must be willing to obtain during onboarding if not certified)
- Minimum 3 years of current nursing experience within the last 6 months
- Willingness to float to units with experienced staff
- Covid-19 vaccination required (declinations/exemptions not accepted)
- Local residents within 50 miles of the facility will not be accepted; own reliable transportation required
- Prior travel experience strongly preferred; rural/small-facility experience highly preferred
- Ability to work in a rural, small-facility environment and manage unit responsibilities under the direction of the Nurse Manager/Supervisor
