Responsibilities
- Provide direct patient care to residents in a long-term care setting, administering medications and treatments per orders and policies.
- Monitor patient status, perform regular assessments, document findings, and communicate changes to the interdisciplinary team.
- Administer prescribed medications, manage safety, and implement basic wound care and other routine nursing tasks.
- Assist residents with activities of daily living, ensuring comfort, safety, and dignity.
- Maintain timely and accurate charting, update care plans as conditions change, and participate in care conferences as required.
- Respond promptly to resident needs and call bells, escalate concerns to the supervising clinician when appropriate.
Requirements
- Active LPN licensure in New Hampshire.
- Availability for 12-hour shift blocks and the stated schedule (Mon, Tue and every other weekend as reflected by scheduling).
- Local to the Lebanon, NH area or reliable local commute.
- Experience in a long-term care setting is preferred.
- Ability to work effectively in a geriatric, long-term care environment and adhere to facility policies.
