Responsibilities
- Deliver telehealth nursing care and assessments for assigned patients, following established standards and the nursing process.
- Administer medications, monitor patient status, manage symptoms, and update care plans as needed.
- Collaborate with the patient, family, and interdisciplinary team to ensure safe, effective, and compassionate care.
- Participate in discharge planning and coordinate follow-up care, referrals, and caregiver education.
- Supervise and direct activities of nursing staff and other care team members; delegate tasks appropriately.
- Document clinical information in the electronic medical record (EPIC) and ensure compliance with organizational policies.
Requirements
- Current Registered Nurse license in the state of practice (California)
- Current Basic Life Support (BLS) certification from an approved vendor
- At least two years of recent nursing experience in the field
- EPIC experience required
- Bachelor’s Degree in Nursing (BSN) preferred
- Ability to work in a travel assignment and in a home hospice telehealth setting
