Responsibilities
- Provide direct nursing care to residents within the scope of practice for an LPN in a long-term care setting
- Monitor vital signs, administer prescribed medications, document accurately, and report changes promptly
- Perform basic nursing procedures, wound care, and assistance with activities of daily living
- Support safety, fall prevention, and independence while maintaining residents’ dignity and comfort
- Collaborate with registered nurses, physicians, and the interdisciplinary team; participate in rounds and care planning
- Complete timely charting and ensure compliance with standards of care and regulatory requirements
Requirements
- Active LPN license in Pennsylvania (or primary state of licensure with eligibility for PA practice)
- CPR/BLS certification
- 1+ year of practical nursing experience, preferably in long-term care or geriatric settings
- Ability to work evenings and commit to 40 hours per week
- Strong documentation and communication skills; comfortable using electronic health records
- Demonstrated teamwork and ability to function effectively in a multi-disciplinary care environment
