Responsibilities
- Plan and conduct segments of physical therapy treatment programs designed to restore and improve physical function, while aligning with department objectives.
- Perform evaluations and establish individualized treatment plans, selecting appropriate interventions and documenting progress.
- Deliver therapy interventions using established protocols and techniques, adapting as patient needs evolve.
- Collaborate with an interdisciplinary team to coordinate care and communicate plans to patients and families.
- Develop and communicate individualized care and discharge plans; ensure comprehensive clinical documentation within established timelines.
- Supervise assigned assistants as needed and participate in ongoing quality improvement activities related to therapy services.
Requirements
- Doctor of Physical Therapy (DPT) degree (required; see note on licensure timing) or foreign equivalent.
- Current Physical Therapy licensure in the state(s) of practice.
- At least two years of recent experience in the field.
- BLS or CPR certification from an approved vendor (per policy).
- Valid driver's license; must be at least 21 years of age.
- Must know HCHB and OASIS; 1 year of home health experience preferred.
- EPIC experience.
- Must be able to work in a hospital-based setting and meet standard credentialing requirements.
