Responsibilities
- Deliver direct patient care to residents in a long-term care setting, including health monitoring and routine assessments
- Administer medications and closely follow physician orders, documenting all interventions accurately
- Monitor vital signs, respond to changes in condition, and escalate to the supervising nurse as needed
- Collaborate with RNs, CNAs, and other team members to implement and update individual care plans
- Maintain clear and compliant documentation in the electronic medical record; ensure accurate recording of care provided
- Adhere to infection control, safety protocols, and dementia-care guidelines; promote resident safety and comfort
Requirements
- Active LPN license in Vermont (or eligibility to practice in VT)
- BLS certification through the American Heart Association
- Minimum 3 years of long-term care experience; prior travel experience preferred
- Associate degree or diploma in Practical Nursing from an accredited program; in good standing with the licensing board
- Ability to work the 3x12-night schedule in a long-term care setting; comfortable with dementia populations
- Willingness to complete required background checks: 7-year nationwide; county searches; FACIS-3; and VCIC within specified timeframes
- Floating to other units is not required for this assignment
