Responsibilities
- Provide direct and indirect nursing care for residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status; communicate findings to the RN and the interdisciplinary team.
- Contribute to an age-appropriate, culturally relevant plan of care; involve the resident and family in care planning; promote a safe environment.
- Recommend revisions to the plan of care after evaluating resident outcomes and coordinate referrals to ensure comprehensive care.
- Maintain current knowledge and competency in nursing practice; participate in mentoring and orientation of staff; uphold safety and policy standards.
- Demonstrate leadership within the professional practice setting under the direction of the RN; apply infection control and standard precautions.
Requirements
- NY Licensed Practical Nurse license (active) and BLS certification (through American Heart Association or RQI).
- Minimum 1 year of experience as an LPN in a long-term care setting.
- Education: graduation from an approved practical nursing program.
- Ability to work in a geriatric, long-term care environment; comfortable with rotating day/evening shifts and weekend/holiday coverage.
- Demonstrated ability to collect, interpret, and communicate patient data; collaborate effectively with the interdisciplinary team.
