Key Responsibilities
- Provide comprehensive LTAC care for medically complex patients, including stable management of chronic conditions, post-acute stabilization, and prevention of preventable complications.
- Lead ventilator weaning processes, tracheostomy care, airway management, and comprehensive respiratory support in collaboration with respiratory therapists.
- Deliver IV therapy, wound care, wound VAC management, and precise medication administration, ensuring safety and efficacy at all times.
- Monitor multi-system status (cardiac, pulmonary, neurologic, renal) with proactive bedside assessment, early identification of deterioration, and rapid escalation when needed.
- Develop, implement, and update individualized care plans in partnership with physicians, pharmacists, case managers, therapists, and family members.
- Coordinate discharge planning, referrals, community resources, and post-acute care transitions to optimize recovery and minimize readmissions.
- Educate patients and families about diagnoses, treatments, medications, and self-care strategies to empower ongoing recovery at home.
