Responsibilities
- Provide direct nursing care to residents in a long-term care setting, delivering hands-on assessment and interventions
- Administer medications, wound care, and other treatments per physician orders and facility policy
- Monitor vital signs, observe changes in resident condition, document accurately, and communicate findings to the care team
- Collaborate with the interdisciplinary team to develop and adjust care plans; participate in rounds and care conferences
- Assist residents with activities of daily living; support mobility, hydration, nutrition, and safety
- Maintain infection control standards, charting accuracy, and regulatory compliance
Requirements
- Active LPN license in Vermont in good standing
- Experience in long-term care or geriatric care preferred
- Current CPR/BLS certification or equivalent
- Ability to work in a fast-paced long-term care setting; good communication and teamwork skills
- Willingness to participate in a travel assignment with a flexible shift schedule
- Ability to pass pre-employment checks and credentialing requirements
