Responsibilities
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and care team.
- Contribute to the development and revision of an age-appropriate, culturally relevant plan of care involving residents/families and the Interdisciplinary Team.
- Provide direct and indirect skilled nursing care under the supervision of a Registered Nurse; maintain patient safety and quality of care for adult and geriatric residents.
- Collaborate with the Interdisciplinary Team; participate in mentoring and orienting staff; maintain current nursing knowledge through continuing education.
- Ensure accurate documentation, adherence to policies, and safe practice; uphold safety and infection-control standards; respond to bloodborne pathogen exposure risks as required.

Requirements
- Current New York LPN license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year LPN experience in a long-term care/ skilled nursing setting
- Ability to provide care to adult and geriatric residents
- Education: Licensed Practical Nurse license
- Availability to stay through the holiday season (end date flexible)
