Responsibilities
- Provide direct, skilled nursing care to patients in their homes per physician orders and agency guidelines

- Document care activities accurately in the electronic health record and communicate changes to the interdisciplinary team
- Educate patients and families on disease management, medications, wound care, safety, and follow-up instructions
- Collaborate with the care team to coordinate referrals, discharge planning, and transitions of care
Requirements
- Active Registered Nursing license in California
- Ability to work in a home health setting and travel to patient homes as part of the assignment
- Strong clinical documentation and communication skills
