Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives.
- Develop and implement individualized PT treatment plans for home-based patients, adapting as needed to progress and home environments.
- Provide therapeutic interventions (e.g., therapeutic exercises, gait training, manual therapy) and monitor client response throughout the course of care.
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating with nurses, physicians, social workers, and other team members.
- Educate patients and caregivers on home exercise programs, safety, fall prevention, and activity modification to support continued progress at home.
- Document assessments, treatment plans, progress notes, and discharge recommendations consistent with regulatory and organizational standards.
Requirements
- Active Physical Therapist license to practice in Oregon or eligibility for Oregon licensure.
- Ability to travel to patient homes and work effectively in a home health/hospice setting.
- Prior home health or hospice PT experience is preferred.
- Strong written and verbal communication skills; ability to collaborate with an interdisciplinary team.
- Capacity to manage a caseload in a travel assignment and maintain accurate documentation.
- Commitment to patient-centered care and safety in the home setting.
