Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team.
- Contribute to a developmentally appropriate, culturally relevant plan of care reflecting nursing diagnosis and interventions; involve the resident/family and healthcare team to achieve outcomes.
- Recommend revisions to the plan of care based on evaluation of resident outcomes and coordinate referrals to ensure comprehensive care.
- Maintain current knowledge and competency in nursing practice; participate in mentoring/orientation of staff; provide leadership in the professional practice setting under the direction of the RN.
Requirements
- Active NY LPN license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience in a Long-Term Care setting
- Completion of an approved LPN program
- Ability to provide care to adult and geriatric residents and work the stated shift, including evenings with EOW/holidays as required
