Responsibilities
- Provide direct basic nursing care to residents under supervision of the R.N. and according to care plans
- Administer prescribed medications and document administration accurately
- Monitor residents’ vital signs, assess condition changes, and report to the supervising nurse promptly
- Assist with activities of daily living and mobility, promoting safety and independence
- Perform routine assessments and document findings; contribute to care planning and updates
- Maintain a clean, safe, and compliant care environment; adhere to infection control and safety protocols
Requirements
- Active LPN license in good standing for the state of assignment (Vermont preferred)
- Current Basic Life Support (BLS) certification
- Prior long-term care or skilled nursing experience preferred
- Ability to work weekends and a 40-hour week in a 7:00–15:30 schedule
- Strong communication, documentation, and teamwork skills
- Ability to work in a fast-paced, patient-centered setting while maintaining professional composure
