Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the interdisciplinary team
- Contribute to development of resident care plans that are developmentally appropriate and culturally relevant, involving residents, families, and team members
- Monitor resident outcomes and recommend revisions to the plan of care; facilitate referrals to ensure comprehensive care
- Maintain current knowledge and competence in nursing practice; participate in mentoring and in-service programs; uphold safety and quality standards
- Deliver care to geriatric residents and maintain professional practice standards within the scope of nursing practice

Requirements
- Active New York LPN licensure
- BLS certification (through AHA or RQI specifically)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to work with adult and geriatric populations
- Availability for Every Other Weekend (EO W) and Holidays as required
- Must be able to start orientation on a Tuesday or Thursday for full-shift onboarding
- Demonstrated knowledge of growth and development in the life span and the ability to assess data relevant to patient status
