Responsibilities
- Assess home health and hospice patients in stable and emergent situations; identify physical, psychosocial, and environmental needs; document findings and coordinate with the interdisciplinary team and referring agency.
- Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care; update the POC as patient status changes.
- Maintain up-to-date patient records, ensuring visits, contacts, problems, plans, actions, and goals are accurately documented and changes are reflected promptly.
- Educate patients and families on care, medications, home safety, and self-care; tailor teaching to age, condition, culture, and learning readiness.
- Supervise and provide clinical direction to HHA/STNA/LVN to ensure quality and continuity of service; support effective delegation and care coordination.
- Manage pain assessment and management; coordinate referrals and participate in on-call responsibilities and interdisciplinary team meetings as needed.
Requirements
- Active Registered Nurse license in the state of Arizona.
- CPR certification – Required.
- OASIS experience – Required.
- Homecare Homebase software experience – Required.
- RN clinical care experience: 1-2 years; Home health experience: 2+ years – Required.
- Education: ADN or Nursing Diploma (required); BSN preferred; CHPN certification preferred.
