Registered Nurse – Home Health travel assignment in Tucson, Arizona. Start date: 05/18/2026. Duration: 14 weeks. Guaranteed 40 hours per week with 8-hour shifts. Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify needs; document findings in the clinical record and team communications.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Gather data on physical, psychological, social, spiritual, and cultural factors; incorporate findings into the POC.
- Maintain current patient records, ensuring visits, problems, plans, actions, and goals are accurately reflected and updated.
- Provide care using strict infection control standards; educate patients and families on care, medications, safety, and self-care practices.
- Supervise and direct HHA/STNA/LVN staff to ensure consistent, high-quality service; participate in interdisciplinary team meetings as needed.
- Assess and reassess pain; apply appropriate pain management techniques and educate patients/families.
- Coordinate referrals to appropriate services and monitor third-party payer requirements; respond to on-call needs as assigned.
- Maintain clear, timely communication with patients, families, team members, physicians, and other health professionals.
Requirements
- Active Registered Nurse license in Arizona; required
- 2+ years of home health nursing experience; required
- OASIS experience and Homecare Homebase software experience; required
- Current CPR certification; required; CHPN certification preferred
- Graduation from an accredited ADN or Nursing Diploma program; BSN preferred
- Current driver's license and reliable transportation; required; ability to travel locally
