Responsibilities
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
- Monitor and document vital signs and changes in resident status; communicate with the care team
- Assist with mobility, transfers, positioning, and safe patient handling
- Support residents with meals, hydration, and oral intake; document intake/output as required
- Maintain accurate charting and care documentation in the patient record
- Uphold infection control practices and maintain a clean, safe environment
Requirements
- Active Certified Nursing Assistant license (state-specific as applicable)
- Current CNA certification; ability to follow care plans and documentation requirements
- Ability to work 12-hour shifts; flexible schedule to meet patient needs
- Prior experience in a skilled nursing facility or long-term care setting preferred
- Able to pass health screenings, immunizations, and background checks as required
- Strong communication and teamwork skills; patient-centered approach
