Responsibilities
- Administer prescribed medications and document administration accurately in resident records
- Monitor residents for changes in condition and escalate concerns to the supervising nurse
- Assist with activities of daily living (ADLs) and provide comfort and support to residents
- Collaborate with the interdisciplinary care team to update and implement care plans
- Maintain clear, timely nursing documentation and ensure chart accuracy
- Adhere to safety, infection control, and facility policies; promote resident dignity and quality of life
Requirements
- Active Licensed Practical Nurse (LPN) license
- Experience in long-term care or assisted living settings preferred
- Strong clinical documentation and communication skills
- Ability to work in a fast-paced, resident-centered environment
- Eligibility to work in the United States; Vermont licensure requirements apply for practice in this assignment
