Responsibilities
- Administer a range of APTA-standard physical therapy treatments to address disabilities, injuries, and diseases
- Relieve pain, restore motor function, and maintain optimal performance for patients in a home hospice context
- Work under limited supervision, documenting patient evaluations, progress, and treatment plans
- Develop and implement individualized treatment plans in collaboration with the care team
- Educate patients and families on exercises, mobility aids, safety, and home exercise programs
- Participate in discharge planning, home safety assessments, and coordination with other disciplines as needed
Requirements
- Active physical therapy license in California or eligibility to obtain licensure
- Knowledge of APTA standard physical therapy treatments
- Ability to work in a home healthcare - hospice setting with patients in their residences
- Strong communication and collaboration skills for working with patients, families, and the care team
- Availability to work 40 hours per week on day shifts
- Completion of credentialing/verification processes as required
