In this role you’ll conduct comprehensive psychosocial assessments, plan and coordinate discharges, intervene in crises, and collaborate with an interdisciplinary team to ensure safe transitions from hospital to home or to community supports. You’ll help patients and families navigate benefits, housing, transportation, and community resources, while aligning care plans with patient goals and values. There are clear paths for professional growth, including expanded social work responsibilities and pathways to leadership or specialization. Our compensation package is competitive and includes a sign-on bonus, housing assistance, and extension opportunities if both you and the facility wish to continue. Weekly pay ranges from $2,381 to $2,419, and you’ll have guaranteed 40 hours per week for a defined term starting 03/23/2026. You’ll work with a dedicated team that offers 24/7 support while you travel, ensuring you have guidance, resources, and backup whenever needed.
