Physical Therapist – Outpatient (Travel)

Shift: 5x8 Days (08:00–16:30) | Setting: outpatient physical therapy with minimal acute inpatient coverage as needed
This travel assignment is with Healthforce Healthcare LLC. The role involves outpatient PT responsibilities with occasional, limited inpatient coverage, within a rural or smaller-facility setting.

Responsibilities
- Deliver comprehensive outpatient physical therapy evaluations and develop individualized treatment plans
- Implement evidence-based PT interventions, therapeutic exercises, modalities, and functional training
- Monitor patient progress, re-assess goals, and adjust care plans accordingly
- Document evaluations, treatment plans, progress notes, and discharge summaries per policy
- Educate patients and families on home exercise programs and self-management strategies
- Collaborate with multidisciplinary team and participate in care coordination and discharge planning
Requirements
- California license: active Physical Therapist license required
- BLS certification required
- Specialty experience: outpatient physical therapy experience required
- Ability to work in an outpatient rehabilitation setting with optional limited inpatient exposure
- Start date alignment with 07/20/2026; 13-week assignment duration
- Education: licensure appropriate to practice as a PT in California
