Responsibilities
- Conduct comprehensive OT evaluations and ongoing assessments for residents in the LTC setting.
- Develop, implement, and monitor individualized treatment plans focused on activities of daily living and functional mobility.
- Select, apply, and progress therapeutic interventions and modalities; document progress and adjust plans accordingly.
- Collaborate with a multidisciplinary team to optimize care, support discharge planning, and coordinate therapy services.
- Ensure timely documentation in electronic medical records and maintain compliance with applicable regulations.
- Provide guidance to care teams and participate in patient/family education as appropriate.
Requirements
- Active occupational therapy license in Pennsylvania (or eligibility for licensure in PA).
- Experience or demonstrated interest in long-term care / SNF settings is preferred.
- Ability to manage a 13-week travel assignment and adapt to facility processes.
- Strong documentation skills and professional communication with residents, families, and the care team.
- Ability to meet background check and health clearance requirements as part of the placement.
