Responsibilities
- Assess residents to determine physical therapy needs and develop individualized plans of care
- Implement therapy interventions including therapeutic exercises, gait training, balance activities, transfers, and functional mobility training
- Reassess progress, document outcomes, and adjust plans of care in collaboration with the care team
- Educate residents and family members on home exercise programs and safety strategies for functional independence
- Maintain accurate, timely documentation in accordance with regulatory and facility policies
- Collaborate with physicians, nurses, and other professionals to support discharge planning and continuity of care
Requirements
- Active Physical Therapist license in the state of New Jersey
- Doctor of Physical Therapy (DPT) degree or equivalent (standard credential for the role)
- Ability to provide care in a geriatric rehabilitation setting within a skilled nursing facility
- Strong written and verbal communication skills; ability to work within an interprofessional team
- Willingness to work a 5x8 day schedule and travel as needed for assignment
- Credentialing and background checks required prior to start
