Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
- Assess patient conditions, implement care plans, and monitor responses to therapies and interventions.
- Supervise and direct activities of various levels of assigned nursing staff; assign tasks and ensure safe delivery of care.
- Coordinate care with interdisciplinary team members and other disciplines to optimize patient outcomes.
- Apply critical thinking, professional judgment, and independent decision-making in clinical situations.
- Document care activities accurately and communicate changes in patient status to the care team and physician.
Requirements
- Current RN license to practice in California (active and in good standing).
- ICU nursing experience or demonstrated proficiency in critical care settings.
- Ability to work 12-hour night shifts and meet the demands of a high-acuity population.
- Strong clinical assessment, patient advocacy, and communication skills.
- Capacity to function effectively in a home healthcare - hospice environment and coordinate with interdisciplinary teams.
- Education: Degree from an accredited nursing program (RN).
