Responsibilities
- Deliver coordinated nursing care for a patient or assigned group in a home hospice setting, following established standards and the nursing process.
- Monitor and assess patient status, document findings, and adjust care plans to meet changing needs.
- Administer medications and therapies, manage IVs and devices, and monitor for adverse effects.
- Collaborate with patients, families, and the interdisciplinary team to ensure safe, effective, and compassionate care.
- Supervise and direct activities of licensed and unlicensed staff as appropriate; coordinate duties within the care team.
- Participate in discharge planning and provide patient/family education to support continuity of care.
Requirements
- Active California RN license required
- Current BLS/CPR certification from an approved vendor
- ACLS certification required
- Hospital Fire and Life Safety (HLFS) certification required
- EPIC electronic medical record experience required
- Minimum 2 years of recent nursing experience; BSN preferred
