Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings and share with the interdisciplinary team.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Collect data on physical, psychological, social, spiritual, and cultural factors and incorporate these into the Plan of Care.
- Maintain up-to-date patient records, ensuring visits, contacts, problems, plans, actions, and goals are clearly stated and updated as changes occur.
- Practice infection control measures to protect patients and staff; educate patients and families on care, medications, home safety, and self-care.
- Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure quality and continuity of service.
- Assess and reassess pain; implement appropriate pain management strategies and educate patients and families.
- Arrange referrals to appropriate agencies when needs exist.
- Monitor assigned cases for third-party payer compliance and documentation requirements.
- Perform on-call responsibilities and provide on-call services as assigned.
- Maintain effective communication with patients, families, team members, physicians, and other health care professionals.
- Participate in interdisciplinary team meetings and other relevant meetings.
- Perform additional duties as assigned or requested.
Requirements
- Current RN license in North Carolina (active)
- OASIS experience required
- Homecare Homebase software experience required
- CPR certification required
- CHPN certification preferred
- Current driver’s license and reliable transportation with valid auto insurance
- 1–2 years of RN clinical care experience; 2+ years of home health experience
- Education: Associate Degree in Nursing or Nursing Diploma required; BSN preferred
- Ability to work in a home care/hospice setting with flexible, locally travel requirements
