Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
- Implement PT interventions within a home health context and document patient response and progress.
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating with other clinicians to align treatment goals.
- Assess patient needs, develop individualized plans of care, monitor outcomes, and adjust intervention strategies as appropriate.
- Educate patients and families on safe home exercise, activity modification, and home safety to support ongoing rehabilitation and independence.
Requirements
- Ability to work effectively in a home health setting and participate in an interdisciplinary care team.
- Strong clinical reasoning to plan, implement, and modify therapy interventions.
- Excellent communication skills for interactions with patients, families, and other care team members.
- Availability to fulfill a 13-week assignment with a 40-hour work week (days).
- Commitment to accurate documentation and adherence to regulatory and professional standards.
