Responsibilities
- Plan and conduct segments of physical therapy treatment programs to restore and improve physical function and independence.
- Implement and adjust interventions to support patient goals in a home health/hospice context.
- Coordinate with the interdisciplinary team to align therapy goals with overall patient care plans.
- Monitor and document patient progress, adjust treatment plans, and communicate outcomes to the care team.
- Educate patients and family caregivers on exercises, safety, and home program adherence.
- Participate in group-based patient activities as a co-leader alongside the care team when applicable.
Requirements
- Professional degree in physical therapy and current licensure to practice in California.
- Ability to work in a home health/hospice setting and engage patients in their home environments.
- Experience or comfort collaborating in interdisciplinary teams and leading patient groups when appropriate.
- Strong communication skills and documentation accuracy for care plans and progress notes.
- Travel-ready with flexibility to accommodate patient needs; 13-week assignment with dayshift schedule (40 hours per week typical, depending on patient volume).
- Education: Degree in physical therapy from an accredited program; Doctor of Physical Therapy degree preferred.
