Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, aligning with department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating care with teammates across disciplines.
- Perform PT evaluations, develop and implement treatment plans, and document patient progress.
- Create and monitor in-home exercise programs and rehabilitation strategies tailored to each patient.
- Educate patients and caregivers on safety, activity modifications, and home exercise programs.
- Monitor patient response to treatment, adjust plans as needed, and ensure safety and quality of care in the home setting.
Requirements
- Active physical therapy licensure to practice (state not specified in notes)
