Responsibilities
- Administer medications and treatments per orders; monitor patient status and respond to changes in condition
- Perform phlebotomy, EKGs, IV starts and IV pump management as needed to support post-acute care
- Provide direct nursing care for post-acute skilled nursing rehab patients; assist with transfers and transports within the facility
- Document care in the electronic medical record; communicate with the unit leader/DO/N and interdisciplinary team
- Collaborate with LNA/CNA/PCT staff to support care plans and deliver dementia-capable, safe patient care
Requirements
- License on file at time of submission
- Basic Life Support (BLS) certification
- Minimum 1 year of clinical experience; 1+ year in post-acute/skilled nursing rehab preferred
- Prior travel experience required
- EPIC EMR experience within the past 2 years
- Dementia-capable care certification preferred
