Responsibilities
- Plan and conduct segments of outpatient physical therapy treatment programs to restore and improve physical, social, and mental functions while aligning with department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating with team members to optimize recovery.
- Assess patient status, establish treatment goals, implement plans of care, and monitor progress toward functional outcomes.
- Document evaluations, treatments, progress notes, and outcomes in the medical record with accuracy and timeliness.
- Educate patients and families on home exercise programs, safety considerations, and activity modifications.
- Collaborate with physicians, nurses, and other rehabilitation professionals to coordinate care and adjust plans as needed.
Requirements
- Active California physical therapy license or eligible for licensure (as applicable)
- Doctor of Physical Therapy (DPT) or equivalent degree
- Outpatient physical therapy experience preferred
- Ability to work 40 hours per week in a day shift
- Comfortable collaborating within an interdisciplinary team
- Ability to work in a hospital-based outpatient setting
