Responsibilities
- Provide bedside nursing care to residents per the plan of care and physician orders
- Administer medications and treatments, document in the medical record, and monitor for adverse reactions
- Assess and report changes in resident condition to the supervising RN
- Perform routine nursing duties, including ADLs support, wound care, and basic rehabilitative activities as directed
- Collaborate with the interdisciplinary team to coordinate comprehensive resident care
- Maintain infection control standards, patient safety, and resident confidentiality
Requirements
- Active Licensed Practical Nurse license in Wisconsin
- Ability to work in a long-term care setting and participate in a 16-hour shift schedule with weekend coverage
- Effective communication and teamwork skills
- Basic documentation and charting proficiency
