Responsibilities
- Deliver RN-level case management and care coordination for patients within the acute care pathway.
- Assess patient needs, develop and implement comprehensive care plans, and monitor progress toward defined outcomes.
- Coordinate discharge planning and post-discharge services, collaborating with physicians, social work, and other members of the care team.
- Maintain accurate, timely documentation in the electronic health record, including care plans, progress notes, and discharge summaries.
- Communicate with patients and families about care options, contraindications, and available community resources; ensure safe transitions of care.
- Monitor for risks such as length of stay and readmission; escalate issues per policy and collaborate to optimize patient outcomes.
Requirements
- Active Registered Nurse license in California (Required)
- BLS certification (Required)
- Minimum 2 years of RN experience in the submitted unit specialty within the last 3 months
- Valid driver's license, current automobile insurance, and reliable vehicle; must be 21 years of age or older
- Ability to work in a short-term acute care setting; strong communication and interdisciplinary collaboration skills
- Education: RN degree as required for licensure
- Eligibility to travel and meet assignment start date and duration
