Responsibilities
- Conduct comprehensive evaluations of residents to determine physical therapy needs and develop individualized plans of care.
- Implement evidence-based PT interventions (gait training, therapeutic exercises, manual therapy, and modalities) to address functional deficits and improve mobility.
- Monitor progress, modify treatment plans as needed, and document outcomes in the resident’s medical record.
- Collaborate with an interdisciplinary team to support discharge planning, rehab goals, and safety strategies for daily living activities.
- Educate residents and families on home exercise programs, gait safety, and fall prevention; ensure patient and family understanding of the plan of care.
- Maintain compliance with facility policies, payer requirements, and regulatory standards; uphold patient safety and ethical practice in all encounters.
Requirements
- Active physical therapy license in Maine or eligible for licensure in Maine.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- 1+ year of physical therapy experience in a skilled nursing or geriatric rehab setting preferred.
- Ability to work on-site in a skilled nursing facility environment and to communicate effectively with the care team.
- Strong documentation skills and comfort using electronic health records to track progress and outcomes.
