Responsibilities
- Assess and evaluate residents' physical therapy needs; perform initial and ongoing evaluations; develop individualized treatment plans with measurable functional goals; coordinate with the care team to align with discharge planning.
- Deliver PT interventions including gait training, balance training, strengthening, therapeutic exercises, ROM, and functional mobility tasks; utilize appropriate assistive devices and adaptive equipment.
- Monitor patient progress using standard measures; adjust treatment plans as needed; document progress and communicate changes to the supervising PT and interdisciplinary team.
- Document comprehensively in the electronic medical record; ensure compliance with HIPAA, regulatory standards, and facility policies; prepare discharge instructions and home exercise programs.
- Collaborate with physicians, nursing staff, and other therapists (OT, SLP, aides) to optimize outcomes; participate in care planning meetings and educate patients and families.
- Ensure safety, infection control, and adherence to facility policies; promote patient and family education on exercises, activity strategies, and fall prevention.

Requirements
- Active Physical Therapist license in Maine or eligible for licensure by endorsement
- Doctor of Physical Therapy (DPT) or equivalent degree
- Minimum 1 year of PT experience in a skilled nursing facility or geriatric rehab setting
- Ability to work in a skilled nursing facility environment and meet assignment requirements
- Ability to obtain and maintain required credentialing for travel assignments; pass applicable background checks and screenings
- Must be legally eligible to work in the United States
