Responsibilities
- Deliver direct, coordinated nursing care for assigned patients in a home hospice context, following established standards of care and the nursing process.
- Administer medications and treatments, monitor responses, and document findings in medical records.
- Collaborate with an interdisciplinary team to coordinate care across services and communicate changes to physicians, case managers, and family caregivers.
- Supervise and direct activities of various levels of nursing staff on the shift, providing guidance and maintaining policy compliance.
- Apply critical thinking, professional judgment, and independent decision-making to patient needs during night shifts.
- Ensure patient safety, accurate charting, and adherence to infection control and quality standards; clock in to the unit when floating as required.
Requirements
- Demonstrated ability to deliver nursing care per established standards and the nursing process.
- Experience supervising nursing staff and coordinating care with other disciplines.
- Availability to work night shifts on a 3x12-hour schedule.
- Willingness to clock in to the unit being worked when floating per policy.
- Strong critical thinking, clinical judgment, and independent decision-making.
- Ability to work within a home healthcare – hospice setting.
