The LPN (LTC) Day Shift, Colorado is a travel nursing position delivering licensed practical nursing care in a long-term care setting with memory care in Gunnison, Colorado. Responsibilities
- Provide direct nursing care and supervise day-to-day nursing activities during your tour of duty in accordance with current regulations and facility policies.
- Administer prescribed medications and treatments, monitor residents, document care provided, and report any changes to the supervising nurse.
- Assess residents, observe and respond to changes in condition, and communicate with the interdisciplinary team to adjust plans of care as needed.
- Collaborate with physicians, therapists, and other staff to develop and update individualized care plans; participate in rounds and care plan review.
- Implement rehabilitative and restorative nursing interventions; assist residents with activities of daily living and mobility, and monitor progress.
- Ensure safety, infection control, and patient privacy; maintain accurate records and comply with BLS and facility standards.
Requirements
- Active LPN license in the state of Colorado
- Current BLS certification; IV certification is preferred for LPNs
- Minimum 3 years of experience in a hospital, skilled nursing facility, or other comparable setting; training in rehabilitative/restorative nursing practices
- CAPS check required; ability to work in a rural Gunnison location with reliable transportation to shifts
