Responsibilities
- Provide case management support for a defined patient panel within an outpatient urgent care workflow.
- Conduct comprehensive nursing assessments, identify barriers to care, and develop patient-centered care plans in collaboration with the interdisciplinary team.
- Coordinate services across the continuum, including referrals, follow-up plans, and appropriate disposition decisions.
- Educate patients and families on conditions, treatment options, medications, and self-management strategies.
- Document care plans, interventions, and outcomes accurately in the electronic health record; monitor progress and adjust plans as needed.
- Communicate effectively with physicians, social workers, and other care team members to ensure timely, safe, and appropriate patient care.
Requirements
- Ability to work in an outpatient urgent care setting.
- Strong clinical assessment, critical thinking, and communication skills.
- Proficiency with electronic health records and documentation.
- Willingness to participate in a travel assignment with a fixed 12-week duration and 40 hours per week.
- Demonstrated ability to collaborate with a multidisciplinary team and coordinate patient care transitions.
