Responsibilities
- Deliver coordinated nursing care for a patient or group of patients in the home hospice setting according to the nursing process and established standards
- Collect, analyze, and document relevant data; develop and implement an appropriate care plan; monitor progress and adjust as needed
- Administer medications and therapies, manage symptoms, and respond to changes in patient condition with appropriate escalation
- Collaborate with the interdisciplinary team, patients, and families; provide education on disease process, treatment plans, and care needs
- Coordinate discharge planning and referrals, delegating tasks to team members as appropriate; promote a safe, healing, and comfortable environment
- Maintain accurate documentation in the electronic medical record, ensure regulatory compliance, and uphold safety and quality standards

Requirements
- Active RN licensure in the state of practice (California) or equivalent
- Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification from an approved vendor
- Advanced Cardiovascular Life Support (ACLS) certification from an approved vendor
- Hospital Fire and Life Safety (HLFS) certification
- EPIC electronic medical record experience required
- Bachelor’s Degree in Nursing (BSN) preferred
- At least two years of recent nursing experience in the field
- Ability to work in a home healthcare - hospice setting; night shift availability required
