Responsibilities
- Perform comprehensive physical therapy evaluations in patients’ homes and establish individualized plans of care.
- Develop, implement, and modify PT treatment plans focusing on mobility, gait, balance, strength, and functional independence.
- Provide therapeutic exercises, manual therapy, gait training, and patient/family education tailored to home environments.
- Reassess progress, document patient responses, adjust goals, and communicate care plans with the patient, family, and the supervising physician.
- Ensure safe patient handling, home safety assessments, and coordination with other care team members to support discharge planning and continuity of care.
Requirements
- Active physical therapy license in Pennsylvania (or eligibility in PA) and in good standing.
- Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
- Relevant clinical experience; home health experience is preferred.
- Current CPR/BLS certification.
- Ability to work in a home health setting and travel to patient homes; valid driver’s license and reliable transportation.
- Strong communication skills and the ability to work independently as part of a multidisciplinary team.
