Responsibilities
- Provide direct nursing care to assigned residents in the LTC/AL setting in accordance with the plan of care.
- Administer prescribed medications and treatments per physician orders, with accurate documentation.
- Monitor residents’ vital signs and overall status; identify and escalate changes promptly to the supervising RN.
- Assist residents with activities of daily living and mobility, ensuring safety and comfort.
- Maintain clear, thorough documentation in the chart and update care plans as needed.
- Collaborate with the interdisciplinary team and supervise care aides as assigned to ensure quality resident care.
Requirements
- Active Licensed Practical Nurse license in Vermont (or eligibility to practice in Vermont)
- Current CPR/BLS certification
- Experience in long-term care or assisted living settings is preferred
- Ability to work evening and night shifts; demonstrate strong communication and teamwork
- Strong documentation, observation, and clinical judgment skills
- Compliance with state regulations and facility policies for practical nursing
