Responsibilities
- Provide direct and indirect nursing care to residents in a long-term care and rehabilitation environment.
- Collaborate with residents, families, and the care team to implement and revise individualized care plans.
- Monitor residents’ condition, document assessments, and communicate changes to the supervising clinician.
- Perform wound care and other skilled nursing tasks as applicable, with attention to geriatric needs.
- Support residents with Alzheimer’s disease and other dementias through person-centered communication and behavior management approaches.
- Assist with rehabilitation-related activities and mobility under the care plan; support safety and falls prevention.
- Maintain accurate records, administer basic treatments per plan, and ensure adherence to clinical policies and regulatory requirements.
Requirements
- Active Maine Licensed Practical Nurse license (State of Maine or compact license)
- Education: Associate degree in Nursing or completion of a Maine State Board approved LPN program
- Experience: 1–3 years of prior RN/LPN experience preferred
- Residency: Candidate must reside outside a 50-mile radius
- Skills: Medical terminology preferred; strong teamwork and communication; ability to float to other units and cover a 24/7 schedule as needed
- Additional: Flexibility to work across unit types and participate in a 24/7 operations model
