Responsibilities
- Provide excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program.
- Act as a liaison between the patient, family, nurse, physicians, multidisciplinary team and the patient’s healthcare benefactor to optimize outcomes.
- Serve as a consultant to the healthcare team on specific patient items.
- Coordinate a continuum of care for a defined population from pre-admission through post discharge.
- Ensure that patients and families proceed efficiently through the course of hospitalization.
- Document and communicate care plans and progress to support quality metrics and care transitions.
Requirements
- Active, unrestricted RN license in California.
- Experience or demonstrated ability in case management or care coordination.
- Ability to work in an inpatient care setting and collaborate with multidisciplinary teams.
- Strong communication and interpersonal skills.
- Eligibility for a travel nursing assignment and ability to adapt to a fast-paced care environment.
