Responsibilities
- Provide excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program.
- Act as a liaison between patient, family, nurse, physicians, multidisciplinary team and patient’s healthcare benefactor to optimize outcomes.
- Serve as a consultant to the healthcare team on specific patient items.
- Coordinate a continuum of care for a defined population from pre-admission through post-discharge.
- Ensure patients and families proceed efficiently through the course of hospitalization.
Requirements
- Active registered nurse license in California (CA)
- Current Basic Life Support (BLS) certification
- Prior case management or care coordination experience preferred
- Ability to work in a hospital setting on a travel assignment
- Excellent communication and collaboration skills with multidisciplinary teams
- Strong clinical assessment and care planning abilities
