Responsibilities
- Provide symptom management and comfort care for in-patient hospice patients during night shifts.
- Conduct comprehensive assessments, monitor changes, and update the plan of care in collaboration with patients and families.
- Educate patients and families on goals of care, medication administration, and caregiving strategies; support caregivers during difficult conversations.
- Administer medications, monitor for adverse effects, and ensure safe transitions when planning transfers or discharges.
- Document care activities accurately; communicate patient status to the multidisciplinary team; participate in care conferences as needed.
- Deliver compassionate emotional and spiritual support, honoring diverse cultural and personal preferences; act as an advocate for patients and families.
Requirements
- Ability to work night shifts in an inpatient hospice setting (7pm–7am).
- Strong clinical assessment skills and critical thinking; effective communication with patients, families, and the care team.
- Willingness to participate in an on-call schedule (2 shifts per month).
- Comfort working in a high-acuity, end-of-life care environment and adhering to standard infection control and safety protocols.
